The Checkmate 227 trial randomized patients to combined immunotherapy with nivolumab and ipilimumab versus chemotherapy frontline. The hypothesis was to postpone chemotherapy to second line only.

The primary endpoint of overall survival, analyzed only in PD-L1 positive patients, comparing ipilimumab/nivolumab vs chemotherapy, met statistical significance and clinically meaningful benefit. Superiority of immunotherapy was also met in response rate, progression free survival and duration of response.

In PD-L1 positive patients, there was a third comparator, nivolumab only. The exploratory analysis shows that nivolumab/ipilimumab was superior to nivolumab monotherapy.

Ipilimumab toxicity: In CheckMate 227, ipilimumab was given at low dose of 1mg/kg every 6 weeks, which was highly manageable. Although different in nature, toxicity was comparable in numbers between immunotherapy and chemotherapy.

The NICOLAS trial investigated concomitant application of immunotherapy and radiochemotherapy in early NSCLC and showed not only that this treatment was safe and feasible, but also encouraging PFS and OS results.
